I accept Aetna,  Regence BlueCross BlueShield, and Moda insurance.

If I am out of network with your plan, you may be able to use out-of-network benefits. I can provide a superbill upon request for you to submit to your insurer for possible reimbursement.

You can also pay by cash, check, or credit card at the start of your appointment. I encourage you to contact your insurance company before your first visit to confirm your coverage, expected costs, and any out-of-network benefits.

• Canceled or missed appointment                $80 on average

• Returned check: $25

• Urgent appointment scheduling: $40

• Copying or sending medical records at your request: $40

• Preparing letters, reports, or treatment summaries: $200 per hour
